The Fursan Core private alpha is a confidential, invitation-only product-shaping program. Before any participant receives non-public screenshots, demos, TestFlight access, provider preview, or community access, they review and accept the documents below.

Each document is reproduced in full on its own page so testers can read, save, and print. Acceptance is captured electronically through the alpha application for the short-form NDA, then completed through DocuSign for the full agreement and applicable riders.

Important legal status These documents are the v1 working draft of the Fursan private alpha legal packet. They are intended for counsel review and may be revised before the first signature is collected. Bracketed placeholders such as [Fursan Health, Inc., a Delaware corporation] reflect the planned formation entity and will be replaced with the executed entity once formation is complete. Mandatory state-law rights are not waived. Nothing on these pages is legal advice; if you intend to participate in the private alpha, consult counsel as needed.

Alpha Privacy and Health Data Authorization

Electronic Records and Signature Consent

Provider / Professional Tester Rider

Conditional rider for trainers, PTs, RDs, nutritionists, athletic and team staff, gym/clinic operators, and other professionals. Doc ID FH-ALPHA-PROVIDER-RIDER-v1.

Military Member Personal Capacity Rider

State-Specific Addendum
